Casewell INTRODUCTION Most clinicians are from time to time perplexed about what can and cannot be achieved by prophylactic antimicrobial therapy. Few topics in therapeutics have generated such confusion, controversy, and heated debate. Despite the arguments there seems to be one consensus, namely that antimicrobial prophylaxis is over-used.

Historically there are many examples of improvements in nursing and surgical procedures that have contributed far more to the prevention of hospital-acquired infection than many attempts at antibiotic prophylaxis. The use of antibiotics some years ago in attempts to prevent bacteriuria in catheterised patients now seems naive when one considers the decrease in infection following the introduction of closed catheter drainage 6 • Some clinicians, at least in some London teaching hospitals', apparently still favour antibiotic 'prophylaxis' for their catheterised patients, and are undeterred by the lack of evidence that such regimens are of any value.
